Panel recommends against ECG tests for heart disease

(Reuters) - Testing electrical activity of the heart using an electrocardiogram is unlikely to help doctors figure out who is at risk of coronary heart disease, according to recommendations from a U.S. government-backed panel.

The United States Preventive Services Task Force wrote on Monday that there's no good evidence the test, also known as an ECG, helps doctors predict heart risks any better than traditional considerations such as smoking, blood pressure and cholesterol levels in people with no symptoms.

"It could potentially be helpful if we had evidence that doing a test like an ECG or an exercise ECG would better classify the people who are at high risk" of heart disease, said Joy Melnikow, a member of the task force from the University of California, Davis.

"Then we could intervene more actively with the very high-risk group which has the highest potential to benefit."

The task force, which published a statement in the Annals of Internal Medicine, recommends against ECG screening of people considered at low risk for heart disease and says there's not enough information to rule one way or the other on those at intermediate or high risk.

On the other hand, there are both costs and possible harms associated with screening healthy adults, Melnikow said.

"The concern is that if people are already at low risk of heart disease and they have one of those tests, if they have an abnormality on the test it's more likely it will be a false-positive result," she added.

"But an abnormal result, whether a false positive or true positive, generally leads to additional testing, and it's the additional tests that could have some risk."

While ECGs themselves are inexpensive and non-invasive, the follow-up tests can involve passing a tube through the heart or other more intensive, costly procedures.

The new recommendations are an update of the USPSTF's 2004 guidelines, which also called for no screening in low-risk adults and said there was insufficient evidence to assess the risks and benefits for screening intermediate- and high-risk people.

Melnikow said that instead of asking for tests, people concerned about their heart health can discuss diet, lifestyle and family history risks with their doctor, who can then use a risk calculator to determine their chance of getting heart disease - and treat them appropriately.

"Everyone can benefit from being physically active," she said. "Everyone can benefit from not smoking, and people can discuss with their doctors whether they could benefit from taking medication to lower their blood pressure or lower their cholesterol."

Afghan construction way behind schedule: report

A new US government report has found major reconstruction projects in Afghanistan are so behind schedule they will not yield results before most combat troops leave, The Washington Post said Monday.

The daily said the report by the special inspector general for Afghanistan reconstruction, due to be released Monday, also states that the Afghan government will not have the money or skills to maintain many of the projects.

"Implementing projects that the Afghan government is unable to sustain may be counter?productive" to the US counterinsurgency mission, the inspector general is quoted as saying.

"If goals are set and not achieved, both the US and Afghan governments can lose the populace's support."

The analysis examines the Afghan Infrastructure Fund, which was authorized by Congress in 2010, the paper said. Over the past two years, Congress has invested $800 million into the fund, and the State Department has committed about $1 billion of its funds to related infrastructure programs.

The report comes as NATO countries have already started to withdraw their 130,000 troops after more than 10 years of war and ahead of a 2014 deadline for an end to combat operations.

According to The Post, the report suggests that Washington may have attempted to build too much in a country with limited maintenance resources.

Meanwhile, the US Embassy in Kabul called the document "speculative," the paper said.

Top sports court rejects Irish equestrian's appeal

LONDON ?

An Irish equestrian has lost his appeal to be selected for the Olympic jumping competition.

The Court of Arbitration for Sport says it rejected Denis Lynch's challenge of Horse Sport Ireland because it had no authority to hear the case.

Irish officials had nominated Cian O'Connor to replace Lynch getting one of two entries in individual jumping. Lynch was originally selected then withdrawn after his horse, Lantinus, was disqualified from a recent event because of health issues.

The competition begins Saturday at Greenwich Park.

O'Connor won the show jumping at the 2004 Athens Olympics but was stripped of his gold medal after his horse tested positive for a banned substance.

Romney comments at fundraiser outrage Palestinians

JERUSALEM (AP) ? Mitt Romney told Jewish donors Monday that their culture is part of what has allowed them to be more economically successful than the Palestinians, outraging Palestinian leaders who suggested his comments were racist and out of touch with the realities of the Middle East. His campaign later said his remarks were mischaracterized.

"As you come here and you see the GDP per capita, for instance, in Israel which is about $21,000 dollars, and compare that with the GDP per capita just across the areas managed by the Palestinian Authority, which is more like $10,000 per capita, you notice such a dramatically stark difference in economic vitality," the Republican presidential candidate told about 40 wealthy donors who ate breakfast at the luxurious King David Hotel.

Romney said some economic histories have theorized that "culture makes all the difference."

"And as I come here and I look out over this city and consider the accomplishments of the people of this nation, I recognize the power of at least culture and a few other things," Romney said, citing an innovative business climate, the Jewish history of thriving in difficult circumstances and the "hand of providence." He said similar disparity exists between neighboring countries, like Mexico and the United States.

Palestinian reaction to Romney was swift and pointed.

"It is a racist statement and this man doesn't realize that the Palestinian economy cannot reach its potential because there is an Israeli occupation," said Saeb Erekat, a senior aide to Palestinian President Mahmoud Abbas.

"It seems to me this man lacks information, knowledge, vision and understanding of this region and its people," Erekat added. "He also lacks knowledge about the Israelis themselves. I have not heard any Israeli official speak about cultural superiority."

As criticism mounted while Romney traveled to Poland, campaign spokeswoman Andrea Saul said: "His comments were grossly mischaracterized." The Republican's campaign contends Romney's comparison of countries that are close to each other and have wide income disparities ? the U.S. and Mexico, Chile and Ecuador ? shows his comments were broader than just the comparison between Israel and Palestine.

While speaking to U.S. audiences, Romney often highlights culture as a key to economic success and emphasizes the power of the American entrepreneurial spirit compared to the values of other countries. But his decision to highlight cultural differences in a region where such differences have helped fuel violence for generations raises new questions about the former businessman's diplomacy skills.

As he has at home, Romney in Jerusalem cited a book titled, "Guns, Germs and Steel," that suggests the physical characteristics of the land account for the differences in the success of the people that live there.

"And you look at Israel and you say you have a hard time suggesting that all of the natural resources on the land could account for all the accomplishment of the people here," Romney said, before citing another book, "The Wealth and Poverty of Nations," by former Harvard professor David Landes.

This book, Romney said in Jerusalem, concludes that "if you could learn anything from the economic history of the world it's this: Culture makes all the difference. Culture makes all the difference."

The economic disparity between the Israelis and the Palestinians is actually much greater than Romney stated. Israel had a per capita gross domestic product of about $31,000 in 2011, while the West Bank and Gaza had a per capita GDP of just over $1,500, according to the World Bank.

Romney, seated next to billionaire casino owner Sheldon Adelson at the head of the table, told donors that he had read books and relied on his own business experience to understand why the difference is so great.

His comparison of the two economies did not take into account the stifling effect the Israeli occupation has had on the Palestinian economy in the West Bank, Gaza Strip and east Jerusalem ? areas Israel captured in 1967 where the Palestinians hope to establish a state.

In the West Bank, Palestinians have only limited self-rule. Israel controls all border crossings in and out of the territory, and continues to restrict Palestinian trade and movement. Israel annexed east Jerusalem in 1967, but has invested much less heavily there than in Jewish west Jerusalem.

And although Israel withdrew from the Gaza Strip in 2005, it continues to control access and has enforced a crippling border blockade since the Islamic militant Hamas seized the territory in 2007.

It's true that Israel has logged tremendous achievements, said Abraham Diskin, a political science professor at the Inter-Disciplinary Center outside of Tel Aviv. But "you can understand this remark in several ways," he added. "You can say it's anti-Semitic. 'Jews and money.'"

The World Bank and the International Monetary Fund repeatedly have said that the Palestinian economy can only grow if Israel lifts those restrictions.

"It's Israeli occupiers and Palestinians under occupation, and that's why Palestinians cannot realize their potential," Erekat said.

The breakfast with top donors ? including Adelson, New York Jets owner Woody Johnson and hedge fund manager Paul Singer ? concluded Romney's visit to Israel, the second leg of a three-nation overseas tour designed to bolster his foreign policy credentials.

Standing on Israeli soil for the first time as the GOP's presumptive presidential nominee, Romney on Sunday declared Jerusalem to be the capital of Israel and said the U.S. has promised never to "look away from our passion and commitment to Israel."

The status of Jerusalem is a critical issue in peace talks between the Israelis and the Palestinians.

In Israel, Romney did not meet with Abbas or visit the West Bank. He met briefly with Palestinian Prime Minister Salam Fayyad.

Romney's campaign says the trip, which began in England last week, is aimed at improving the former Massachusetts governor's foreign policy experience through a series of meetings with foreign leaders. The candidate has largely avoided direct criticism of U.S. President Barack Obama while on foreign soil.

The Jerusalem fundraiser, however, was a political event that raised more than $1 million for Romney's campaign. It marks at least the second finance event during his tour. The first, in London, attracted about 250 people to a $2,500-per-person fundraiser.

Both presidential candidates have aggressively courted American donors living abroad, a practice that is legal and has been used for decades.

Romney's declaration that Jerusalem is Israel's capital was in keeping with claims made by Israeli governments for decades, even though the United States, like other nations, maintains its embassy in Tel Aviv.

His remarks on the subject during a speech drew a standing ovation from the audience, which included Adelson, the American businessman who has promised to donate more than $100 million to help defeat Obama.

Adelson was among a several donors who flew to Israel for a day of sightseeing with Romney in addition to private meetings with top Israeli officials.

Romney flew to the Middle East from Britain, where he caused a stir by questioning whether officials there were fully prepared for the Olympic Games. A stop in Poland will complete his trip.

Four years ago, Obama visited Israel as a presidential candidate, part of a five-nation trip meant to establish his own foreign policy credentials.

A goal of Romney's overseas trip is to demonstrate his confidence on the world stage, but the stop in Israel also was designed to appeal to evangelical voters at home and cut into Obama's support among Jewish voters and donors. A Gallup survey of Jewish voters released Friday showed Obama with a 68-25 edge over Romney.

Romney and other Republicans have said Obama is insufficiently supportive of Israel.

Johnson City native Jerry D?Amigo of the AHL?s Toronto Marlies was joined by Philadelphia Flyers? Tom Sestito and others in trading in the sticks for nine irons at En Joie this afternoon.

It?s all for the group called S.C.O.R.E (Skating Community for Opportunities in Recreation and Enrichment), which is raising money to build a local ice rink and community center in Endicott.

?It?s a great cause. It?s for a great thing and I?m glad to be out here golfing and it?s on a beautiful course, so why not,? said D?Amigo.

?Grippen Park provided so much recreation and activities for kids for years here in this part of Endicott, Western Broome. We?re trying to recreate that with our own community grass roots project,? said President Jim Tofte.

The event concluded Friday evening with a block party. Featured auction items included a signed NHL All Star Jersey and autographed Barry Sanders Jersey.

There is no reason to be embarrassed or afraid of living in a sober living Delray Beach community. Similarly, halfway house Delray Beach communities are the same type of system and they are bull very great places to live.The 2013 year is expected to have an increase of people who were scouting out these types of communities. Partly because of the increase in people who are getting addicted to drugs, but also because more people are stepping up and trying to get sober and clean for themselves. Awareness is a big part of drug prevention. From all of the television commercials and types of promotion that are going on to raise drug awareness, people are starting to wake up and realize that they have a problem and need to get help. It's also making it easier for people to not feel so isolated.


When they see commercials on TV, they are able to relate to them and see that there must be other people out there who are in the same position as they are. These commercials provide insight and they are able to encourage drug addicts and people who are really struggling in life to get help and get sober right away. Our efforts to fight the war on drugs may not be successful, but we are making mild achievements. Halfway houses and sober living facilities are a step in the right direction and it is absolutely wonderful that these types of locations are available for addicts and people who are trying to recover.


Ultimately, 2013 should be a profitable year for these types of facilities but it is not something to celebrate. Any time somebody gets addicted to a substance, there is a huge amount of struggle that they will go through in order to recover. It also affects their family and friends significantly.

As it happens, the Tax Policy Center at the Urban Institute and the Brookings Institution evaluated the distributional impact of Bowles-Simpson?relative to current policy. ? Here is what they found:

Understand the role of White Balance in Digital Cameras











White balance is central to photography. It is the way in which the camera captures colors as they are. In theory when you white balance a picture, you essentially ensure that the white in real looks as white in the picture too. What this does is that it enables the digital cameras sensors to interpret other colors correctly. However this is easier said than done. There are various shades of white, some cooler (which render a bluer tint) and others warmer (which render a yellowish tint) that can confuse the camera and you may end up capturing altered colors.

To counter this, modern digital cameras come loaded with White Balance presets most noteworthy of which is the Auto White Balance mode. It is also by far the most commonly used White Balance preset by default. Auto White Balance in digital cameras tries to average out the color temperatures and thus capture an average color picture. This may or may not be to your liking.

So what does one do?

The answer lies in the plethora of other white balance presets such as Cloudy, Shade, Florescent et al. These presets work in an interesting way. While they stand for a certain condition like Shade or Cloudy, they work differently. They try to average out such conditions e.g a Cloudy preset tries to compensate for the bluish tones present in shade and bring in more of warm tone in the picture. A cloudy does the same. Florescent, on the other hand, does the opposite.

The most powerful preset of all is the Custom White Balance preset. It is the most difficult to master as well. To use this preset, you need to select a white or a grey spot in the picture that define the perfect grey and use that as a reference for rest of the colors. Once chosen, the same custom white balance can be used across different images, given that the lighting conditions are the same.

In short, one should remember that role of white balance is to capture colors as the eye sees it. By dialing through the various modes and presets, you can learn what preset suits the conditions and leverage that to capture crisp pictures.

I have plenty of dentistry office to choose from to book an appointment for oral/dental care services. Not only are there several general dentist health care offices, but there are a lot of specialized dentist health care offices.

I?ve enjoyed a life of good dental health care due to past dentistry habits of regular dental health care visits, routine and effective dental health care habits, and great dental health genes. Even in my fantastic dental health, I have needed the dental services of a specialized dentist twice?once for an oral surgeon dentist to repair some of my dental tissues with minor oral surgery services, and the second for oral surgery services to remove my wisdom teeth just for dental preventative care precautions. Both of the dental services I received from a specialized dentist where services I was able to choose after some oral health care counsel from my general dentist office. The general dentist informed by of the reasons why the dental procedures would be a good idea, for preventative dental care measures only to remove the possibility of dental health care problems popping up in the future. If I could prevent these dental problems through less expensive dentist services and very minor oral services now then in the future I wouldn?t have to worry about having to choose major oral surgery or oral health care services from not being able to choose in the past, to do some simple dental procedures.

Dentist health care services are so important to the health of a city and area. It?s not everywhere that you can choose your dentistry from a selection of dentist?s offices. It may be that in an area there is only one local dentist and one local oral surgeon services. It may be that in an area the local dentist cannot perform specialized dental services such as dental implants and the dental patients would have to choose to travel some distance for their most local dentist specialist.

In any case, it is very important to the health of an area to have a dentist and to have those oral health care services. Oral health is linked to good over-all health. I want to create an online bookstore for our current mail order bookstore. I do not need an ebook section. We currently produce a paper catalogue which is mailed to our customers. I need a well designed website with a CMS that allows books to be categorised by catalogue and by category and by both - for example the history books in the July catalogue, would have fewer books than all the history books. The entire website needs to be fully searchable by author, title, ISBN and keywords. We like the look of the website www.readings.com.au but we dont need a music section. Additionally we would like to turn our paper catalogue into a web catalogue that integrates with our shopping cart. similar to the catalogue on http://www.psbooks.co.uk/ Both the website and online catalogue once up and running need to be able to be completly managed by me, including adding and removing content, and uploading new online catalogues.

Entropy can lead to order, paving the route to nanostructures

ScienceDaily (July 26, 2012) ? Researchers trying to herd tiny particles into useful ordered formations have found an unlikely ally: entropy, a tendency generally described as "disorder."

Computer simulations by University of Michigan scientists and engineers show that the property can nudge particles to form organized structures. By analyzing the shapes of the particles beforehand, they can even predict what kinds of structures will form.

The findings, published in this week's edition of Science, help lay the ground rules for making designer materials with wild capabilities such as shape-shifting skins to camouflage a vehicle or optimize its aerodynamics.

Physicist and chemical engineering professor Sharon Glotzer proposes that such materials could be designed by working backward from the desired properties to generate a blueprint. That design can then be realized with nanoparticles -- particles a thousand times smaller than the width of a human hair that can combine in ways that would be impossible through ordinary chemistry alone.

One of the major challenges is persuading the nanoparticles to create the intended structures, but recent studies by Glotzer's group and others showed that some simple particle shapes do so spontaneously as the particles are crowded together. The team wondered if other particle shapes could do the same.

"We studied 145 different shapes, and that gave us more data than anyone has ever had on these types of potential crystal-formers," Glotzer SAID. "With so much information, we could begin to see just how many structures are possible from particle shape alone, and look for trends."

Using computer code written by chemical engineering research investigator Michael Engel, applied physics graduate student Pablo Damasceno ran thousands of virtual experiments, exploring how each shape behaved under different levels of crowding. The program could handle any polyhedral shape, such as dice with any number of sides.

Left to their own devices, drifting particles find the arrangements with the highest entropy. That arrangement matches the idea that entropy is a disorder if the particles have enough space: they disperse, pointed in random directions. But crowded tightly, the particles began forming crystal structures like atoms do -- even though they couldn't make bonds. These ordered crystals had to be the high-entropy arrangements, too.

.Glotzer explains that this isn't really disorder creating order -- entropy needs its image updated. Instead, she describes it as a measure of possibilities. If you could turn off gravity and empty a bag full of dice into a jar, the floating dice would point every which way. However, if you keep adding dice, eventually space becomes so limited that the dice have more options to align face-to-face. The same thing happens to the nanoparticles, which are so small that they feel entropy's influence more strongly than gravity's.

"It's all about options. In this case, ordered arrangements produce the most possibilities, the most options. It's counterintuitive, to be sure," Glotzer said.

The simulation results showed that nearly 70 percent of the shapes tested produced crystal-like structures under entropy alone. But the shocker was how complicated some of these structures were, with up to 52 particles involved in the pattern that repeated throughout the crystal.

"That's an extraordinarily complex crystal structure even for atoms to form, let alone particles that can't chemically bond," Glotzer said.

The particle shapes produced three crystal types: regular crystals like salt, liquid crystals as found in some flat-screen TVs and plastic crystals in which particles can spin in place. By analyzing the shape of the particle and how groups of them behave before they crystallize, Damasceno said that it is possible to predict which type of crystal the particles would make.

"The geometry of the particles themselves holds the secret for their assembly behavior," he said.

Why the other 30 percent never formed crystal structures, remaining as disordered glasses, is a mystery.

"These may still want to form crystals but got stuck. What's neat is that for any particle that gets stuck, we had other, awfully similar shapes forming crystals," Glotzer said.

In addition to finding out more about how to coax nanoparticles into structures, her team will also try to discover why some shapes resist order.

This research was supported by the U.S. departments of Defense and Energy, the Deutsche Forschungsgemeinschaft and the National Science Foundation. The paper is titled "Predictive Self-Assembly of Polyhedra into Complex Structures."

Glotzer is the Stuart W. Churchill Collegiate Professor of Chemical Engineering and a professor of chemical engineering, materials science and engineering, macromolecular science and engineering, and physics in College of Literature, Science, and the Arts.
